When calling for an appointment, please be prepared to provide the patient's name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable.

Directions to office:
We are 45 minutes from Nashville: From Interstate 24, take exit 11. Go on Route 76 west, towards Clarksville. Proceed to Route 41A (also called Madison Street) and turn right. Proceed on Madison St. to the intersection with Memorial Drive. Gateway Medical Center is at this intersection. Turn right onto Memorial Drive. Go .7 mile and turn right onto Landrum Place. We are in the Medical Arts Building, which is the first building on the left.

From Madison Street, Turn north onto Memorial Drive, go .7 miles, turn right onto Landrum Place.
